[GLIMS] Data on "extinct" glaciers sought

Hello GLIMS community,
I've been implementing a layer in the GLIMS Glacier Viewer
(https://www.glims.org/maps/glims) that will show on the map where
glaciers are known to have disappeared.
I am asking you all for data on "extinct" glaciers in your region (any
region you work in). To include into GLIMS, I need:
1. GLIMS Glacier ID
2. Estimated date of disappearance (a date is required, but if only the
approximate year is known, then any suitable date in that year is okay)
3. Uncertainty (plus/minus value, in years) of the date
The focus here is on glaciers that have disappeared in the last several
decades -- not since the last ice age. Below is an example text table
formatted in a way I could accept from you:

If you know of a glacier that is gone but not in GLIMS yet, then contact me
about adding an outline of that glacier (presumably from older imagery) to
GLIMS first.
What does "disappeared" mean? If it used to be a glacier that you would
put in an inventory (and is in GLIMS; see last paragraph), but no longer
is (is really gone or has become a snowpatch), then it should be called
"disappeared".
Timeline:
- Please let me know as soon as possible if you have data to contribute.
- Data is desired by the end of February.
