NOTE: This form should only be used for submitting reports for flights that occurred on or before 2012-09-30.
For any flights that occurred after this date, please use the form found at:
http://espo.nasa.gov/missions/oib/node/add/mission-science-report

indicates a required field.

For NASA or UTIG campaigns, Overall Region can be one of: Greenland or Antarctica. For UAF campaigns, Overall Region should always be "Alaska".
